You need to register for Bihar state counselling when it opens, fill your choices including BPT at IGIMS, and wait for seat allotment.
The course has very few seats, so getting a seat with 452 marks (OBC, Kerala) may be difficult, but you should still apply. If you get a seat, you will have to complete document verification and fee payment.
It is also better to keep other colleges as backup options.

So to participate in the NEET couselling you need to:
1. Fill the application form online through their official website.
2. Register yourself and pay the counselling registeration fee.
3. Do your choice filing properly while registering for counselling.Lock your choices properly.
4. After registering you need to upload your NEET scorecard,class 10 and 12 marksheet,category certificate or other necessary documents.
5. After some days merit list will be released. If your rank matches you will get a seat. If you don't get a seat in first round participate in other.
6. After your seat allotment you need to go to the college physically for documents verification.

The cut-off for BC2 category in Bihar is usually around 550 to 600 marks. Your score is much below the required cut-off.
You are eligible for counselling, but chances of getting an MBBS seat in any government college are almost negligible. You can try for private medical colleges or consider other courses like BDS, BAMS, or B.Sc Nursing.
If you aim for a government MBBS seat, you should prepare and try again next year.

With 279 marks in NEET 2025, OBC category, AIR 5 lakh, and OBC rank 2 lakh, it is very difficult to get B.Sc. Nursing or GNM in IGIMS (Patna) because it is a top government institute and the cut-off is usually much higher. But you can still get B.Sc. Nursing or GNM in other government or private colleges through state-level counselling (like Bihar BCECE UGMAC) or private college quota.
The IGIMS application form for B.Sc. Paramedical (including B.Sc. Nursing, BPT, etc.) is not released separately but is part of the state-level NEET UG counselling managed by BCECEB.
You should apply for BCECEB counselling in June 2025 (post NEET result).
After document verification and merit list publication, you will get to choose IGIMS Patna as a preferred college during choice filling.
