First of all, if you start charging a minimum of USD 20,000 per surgery, then hardly any patient will prefer to go to you , because the average charges which even a reputed and very experienced neurosurgeon charges is much less (around USD 1000-2000). For certain critical neurosurgical procedures (like Spinal Fusion Treatment) you may charge high, but again that depends what qualifications you possess, how much experienced you are and how many successful surgeries you have performed before. A neurosurgeon, who has just entered into profession, if starts charging too much from beginning, no patient will prefer him.
So if I conclude, the first "if" statement is invalid and you just cannot charge USD 20,000 minimum per surgery.
Yes, a neurosurgeon can come to India (because MCI/NMC recognizes USMLE, the Licentiate Exam needed to be a doctor in US). He can definitely establish a Super Speciality Hospital. But the expense you mentioned, which is 300 Crores INR, depend on your financial capabilities.
After opening a hospital, if you are a good reputed neurosurgeon, you can easily earn around INR 20-25 Lakhs per month. (Source - My relative who is a neurosurgeon in Odisha)

In Step 1, you need attempt for 300 MCQs, it is important to score good in the exam, as it shows major impact on your residency programme.
