I don't know which examination you are talking about, but with this rank in any examination be it medical or engineering, getting a seat is very tough. You can apply for the private colleges but also you have little probability to get a good private college.
I would say you should give some other entrances in order to be safe.

------- Those students who have scored Better Marks and All India Ranks are alloted seats first as per their choice.
------ The State in which Medical College is located has no role to play during seat allotment for Medical colleges during AIQ Counseling.
