Yes you can study MBBS under PwD quota because as per the rules if the candidate is suffering from more than or equal to 40% but less than or equal to 80% Dyslexia then he/she can get admission to MBBS course under PwD quota on the basis of NEET UG score, but there is a condition that selection under this category will be based on learning competency evaluated with the help of the remediation/assisted technology/aids/infrastructural changes by the Expert Panel.

Dear candidate, medical certificate can be obtained from any registered medical practitioner. It can be from Medical Officer or Doctor as well, unless there is any special criteria mentioned in the medical certificate criteria of the Institution you can genuinely submit medical certificate obtained from any hospital, any registered medical practitioner.

I am extremely sorry to hear such thing, I hope this doesn't impact your mental health , those who apply under PWD category most of the them have either serious visual or auditory defects or sometimes severe paralysis but if you go through CAT Brochure and you find your above-mentioned diseases to be mentioned in the PWD list , then you can apply. Dyslexia is a neural disorder, so from what I have researched you have to get a certificate from a licensed psychologist or a neurologist.
SIDENOTE: The certificate should mention "severe" category to get the benefit of pwd

To avail the services of scribe, the candidate should request the Chairman, JEE (Advanced) 2020, of the respective zonal coordinating IIT in the prescribed format (FORM-SCRIBE). The requisition, along with a copy of the PwD certificate (FORM-PwD [II/III/IV, whichever is applicable]) OR copy of the certificates of Dyslexic candidate (FORM- DYSLEXIC 1 and DYSLEXIC 2), whichever is applicable, must be uploaded at the time of online registration for JEE (Advanced) 2020. The formats for the request letter and PwD/Dyslexic certificate are given in Annexure-II: CERTIFICATE FORMATS.
Physical Disability certificate (FORM-PwD II/III/IV whichever is applicable]) issued by the notified medical authority. Dyslexic candidate needs to submit FORM-DYSLEXIC 1 and 2 instead of FORM-PwD. The certificate must mention ‘SEVERE’ under Dyslexia category to enable the candidate to get the benefit of PwD category. Individual IITs constitute medical boards at the time of admission to test the candidate’s level of disability. The opinion of such medical board shall be final and on this basis, the benefit of PwD category may be allowed or withheld.
