If you have already been issued the grant certificate of gazette for change in your name then you can fill up the new name, but i guess your last qualifying certificates might be issued in your old name, so it would be recommended to fill up the form

So if you are in class 12 or you have cleared class 12 and you are thinking to take admission in one of the law colleges prefarably NLUs then the eligibility criteria is one should have secured 45% marks in class 12 or equivalent examination for unreserved categories
