Since, you've already have your improvement examination, you should use the Roll number mentioned in the admit card for your improvement examination while filling your neet form.

As per the NEET eligibility criteria, there is no such eligibility restrictions regarding the gap year. So if you are a student of 10+2 having Physics, Chemistry and Biology as subjects and if you have scored at least 50% marks in aggregate, then you are eligible for the NEET examination.
