It won't create any problem but as per the instructions of NTA you should put the same name as mentioned in your class 10th passing certificate so if it consists of your surname then you should enter it and don't worry you can correct it during NEET UG

See as per your Marks-365 it is difficult to get a Government Medical Seat even if you have a state quota and another quota if applicable.

Sorry, There are no chances to get into any medical college with this mark.

Obviously you can get admission in any college after doing diploma and Entry as lateral. As now a day most of the colleges take admission in their college but the condition is only that you have to do give the exam of their college first to get admitted.
