after reviewing previous years cutoff at this score it is really impossible to get mbbs in any government college even for your category. minimum 500+ is needed. and this year cutoff scores have increased also.

SASTRA seems to be a bit tough here given your rank in JEE mains but a cake walk with intermediate GPA. Also depends on what you prefer to have in there too. I suggest you give another try and improve your JEE rank to strengthen your chances at SASTRA. Rest

If they did not ask for the NEET score, then why should you mention it there! Just fill the form according to the instruction. That will be enough.
