It is very difficult to get into a government college at this rank. However, you have the option of going to a private college. You can look for some private colleges like LPU, CU, KIIT, MIT, CMRIT, etc. You can also get admission through the management quota. You can contact the college directly or you can go through counsellors. Management quota is accepted by almost all private engineering colleges.

However, in my opinion, you should take a drop and prepare for next year. Stay consistent and work hard for one year and get into your dream colleges. That will be the best option for you.

Admission in IITs is through jee advanced and not jee main
This difficult to predict the marks/percentile / rank you require in JEE mains to get into NITs etc as closing rank keeps changing every year depending upon various factors like

In Bihar state quota counseling you have less chances to get PMCH ( patna medical College and hospital) as last year cut off marks for Gen category was 635 marks. Here also your chances are there as of now result haven't been declared so don't loose hope. It's the best college in Bihar as it's oldest one.
You can get next best college Indra Gandhi institute of medical science,patna which cut off marks was 628 marks,so you have almost confirm chances here also. So this is the best option for you in Bihar and also it's good college and mostly prefer in every section academic and overall facility ( PMCH is preferred due to old).
You can also get all of the govt medical colleges in Bihar except of PMCH like.
Spending only 4 lakhs in MBBS is totally worth it. This much money you might only earn in first 6-8months after graduating MBBS.

To be honest , being from EWS category you don't have a chance to get into NITs /IIITs with this percentile. As you are from Bihar i would suggest you to look for colleges through your state counselling as that would be much beneficial for you.
Through josaa it is not possible for you to get any seat, i would suggest you to also look for private colleges such as SRM University, Manipal University, BITS Pilani,VIT Vellore.

If you are a PwD(Person with Disability) candidate then there is certain reservation for candidates with PwD in Medical Colleges through NEET.
• 5% of seats are reserved for Person with Disability PwD. This is a horizontal reservation which means that 5% of all seats reserved for various categories are reserved for candidates with disability from that category
