No, you will not be benefitted now with your EWS category. As in JEE Advanced form filling, you removed your category then in your JEE Advanced rank, your category rank is not shown and without category rank, you can't get benefits of the category. Hope this will help you.

If you have given your jee mains as an ews candidate and taking into consideration you have also got your ews rank in JEE mains so let me tell you that you will have the benefit in counselling for the ews category.
But if you haven't got your ews rank in jee advanced then I don't think you would get the benefit for it

Securing anywhere above or between 350-400+/720, you are well posed to secure a seat in BAMS or BHMS at some of the Top Government or State Ayurvedic/Homeopatheic Medical Institutes, with good infrastructure, faculty and training facilities. Since you haven't provided more details regarding which application form you are referring to in your question, I would tell you in general sense. If you've been asked to upload your marksheet in any application form then you have to scan an original marksheet. So, there is absolutely no issue if you have scanned an original marksheet. Just make sure that the scanned image is clearly visible and understandable. In any case, if anyone doesn't have the marksheet which is provided by school then one can provide the DigiLocker downloaded marksheet too.

According to previous year cutoffs trend, there is very low chances for you to get government college for MBBS in your category in Andhra Pradesh. To get government college one must score atleast 540+ marks in NEET UG exam in your category.
Also in private medical colleges you can get admission in MBBS only through management quota. However the cutoff changes every year due to various factors such as number of students participated in counselling and number of students attempted exam, difficulty level of exam, reservations and more. In NEET 2020 kerala state quota cut off marks for EWS category candidates was Air 25114 and 590 marks and state rank 3129.
So if we compare your expected marks then it is way below than previous year cut off marks so there are very rare chance that you will get govt mbbs college in Kerala State counseling.
Although you may get self financing private medical colleges at your marks in keam counseling,you can try for that.

Nothing fixed can be said until NEET 2021 results come out. Cut offs and ranks are subject to change every year depending on the difficulty level of the paper, the number of seats available and the performance of the students.

Last year the cut off of government medical colleges in Uttar Pradesh under EWS category through state quota was a rank of 16979. In 2020, this corresponded to 606 marks while in 2019 this corresponded to marks between 570-561. So based on this you may or may not get government colleges in Uttar Pradesh. But you can surely get management quota seats in private medical colleges of Uttar Pradesh.
