The National Testing Agency (NTA) will hold India's single biggest medical admission exam, the National Eligibility cum Entrance Test (NEET). NTA is yet to declare the date of the NEET-UG 2022 test as well as the application form date information. NEET-UG 2022 is scheduled to be held on any Sunday between the latter week of June and the first week of July, according to sources. However, no word on the NEET 2022 exam date has been received. NEET 2022 will be held in pen and paper format for admission to 89,395 MBBS, 27,948 BDS, 52,720 AYUSH, 603 BVSc, and AH seats, as well as 1,899 AIIMS and 249 JIPMER seats.

As per your nativity your Domicile is Chattisgarh. So you'll be eligible for State Quota benefits in Chattisgarh but not in Jharkhand.
Through State Quota Counselling 85 percentage of State Quota seats and 100 percentage of Private Colleges seats are filled. If you want to apply for Jharkhand Colleges then only possible way is All India Quota NEET UGCounselling.

NEET application form 2022 will be released by NTA in April or may, the official dates have not been released by them yet. The application fees for general candidates is 1500 and for ST/ SC/ PWD/ Transgender category is 800 rupees.

See if you belongs to unreserved/general category then you should at least have 50% aggregate in PCB and if you do not secure this much marks then you can appear for NEET but you will not be eligible to sit for NEET counselling.
For NEET 2022 there are some other criteria which needs to be fulfilled :
** You must have passed 10+2 from any recognized central/state board.
** English should also be in your curriculum of class 12th and also you should have passed in it.
** You minimum age should be more that or equal to 17 years by December 31st 2022, as per the current eligibility criteria.
** As of now there is no upper age limit for NEET but the case is pending in Hon'ble SC and by 2022 if the verdict comes then the upper age limit may be decided and it can be either 25 or as decided by SC.

See NEET UG Application form is usually released by this time but this year it is facing some delay because NEET UG 2021 counselling which is still in process.
So at this moment it would be too early to say any exact date about NEET 2022 but to give you an idea, if everything goes normally then we can expect NEET 2022 application to get started from the month of February/March 2022 and tentatively it would be conducted on very first Sunday of May month, but nothing is announced officially yet.

If you have the domicile certificate of UK and if you satisfy the following conditions then you are eligible to avail of the 85% state quota in Uttarakhand MBBS admission :

Yes you need to choose 12 appearing while filling the Neet 2022 application form.
This because, You are going to appear for 12th NIOS examination 2022 so you need to choose 12th appearing otherwise if you choose 12 passed then you will have to fill your PCB marks in the application and thus you will become ineligible as you don't have 50% aggregate in PCB, assuming you from general category .

You can apply for neet 2022 only if you are eligible for it
To be eligible for neet 2022 one must be either appearing for 12th board examination 2022 or should have passed 12th board examination with physics, chemistry ,biology/biotechnology and English and must have got at least 50 % in aggregate of PCB i.e Physics, Chemistry and Biology /biotechnology in 12th examination if belongs to general category , or at least 40% in aggregate of PCB if belongs to Obc/ Sc /st category. Also, you need to be at least 17 years old as on 31st December 2022.
