You must upload your sc certificate in neet application form . Fill all the important information correctly and must upload necessary documents.
There are lot of benefits to sc candidates in medical admissions through neet .
For general category candidate qualifying cut off are 50 percentile but for sc class there is 5 % relaxation ie 45 percentile . Also there is relaxation in fees too.
Out of 15% All India Quota there are 15% seats reserved for sc category candidates . 7.5 % to ST and 27 % seats are reserved for obc category candidates. EWS are listed to get 10% seats and for PWD it is 5%

NEET eligibility criteria by NTA - Qualifying exam requirements. Candidates must have passed 10+2 or equivalent with Physics, Chemistry, Biology/Biotechnology, and English as core subjects from a recognised board . Class 12 or equivalent appearing aspirants are also eligible to apply for NEET.

Answering to your question, The eligibility criteria for NEET require candidates to meet the following qualifying marks in the 12th board exams:
1. Candidates in the UR category must have secured a minimum of 50% in the aggregate of Physics, Chemistry, and Biology (PCB) and passed each of these subjects individually.
2. For OBC/SC/ST/reserved PH categories, the qualifying percentage is 40%.
3. The unreserved PH category must have a minimum of 45%.
These criteria are applicable to students from all schools and boards across the country. To check eligibility, ensure that your 12th percentage is above 40%.

Candidates must have completed 10+2 in Science stream from any recognised board . They should have completed 10+2 having Physics, Chemistry, and Biology as compulsory subjects. Candidates need to appear for national or university level entrance exams for admission to BPT course.

If tou have completed your schooling and class xii then you must have domicile of Gujarat and sc certificate from Gujarat. Your form in neet may be rejected due to this reason.
To take benefits of Sc categryou must have Sc certificate of that particular state.
Now you belong to Uttar pradesh but you have completed your studies in Gujarat so you kust have applied for Gujarat domicile .
Eligibility for state domicile ( you are consists resident of that state if) is that you or one of your parents must have lived in that particular state for more than 10 years .
One person can't have domicile and sc certificate from two states .
You may contact government officials( Ration office ) for your problems . They will solve your issue .

As per your question, NO there will not be any problem if your mobile number is unlinked from aadhar card, as you might have already provided an alternate number and an email along with the same. So any notifications with respect to the exam can be checked out through the email.
