in neet 2021 application form for 12th / equivalent marks i m from telangana intermediate should i put both 1st and 2nd yr that is out of 1000 or only 2nd yr marks out of 560 i passed in 2018

Let me correct you first that 85% of Govt seats of any state are being covered under state quota so it is not 75% and if you are having the residential proof or the domicile of Maharashtra state then yes, you are eligible to participate in its state counseling.
