As you asked is a 400 above score is good and yes, 400+ is a very good score in KEAM that will get you a very good rank considering the fact that you mentioned that you belong to OBC ezhava category and which is only if you got good Percentage in your board examination which you did not mention.

It is not possible to predict the rank just based on your marks. The rank you obtain in KEAM is based on both your KEAM Score as well as your 12th Class Examination.
Someone might have obtained the same score as you, and in that case your Mathematics section marks are compared followed by Physics section's marks in order to break the tie. So I suggest you to wait for the official rank list.

Private Medical Colleges do not participate in All India Counselling and the seats are filled through State Counselling alone. And for appearing for State Counselling you need to hold the domicile of that particular State. Or else the only other option is through Management Quota. There are 3 Categories in Private Medical Colleges. Category A is the Government Quota Seats. And only candidates of the home State are eligible to apply for these seats. B Category seats are also called Management Quota Seats and they are filled through State Counselling and the remaining seats are filled through direct Admissions by contacting the College directly provided the Candidate has registered for the respective State Counselling and has not been able to secure a seat in the Counselling. Here the fees is usually nothing less than 40 lakhs. The last Category is NRI seats Category and you cannot apply for that. Himachal Pradesh, Haryana, West Bengal, Telangana, Tamil Nadu, Karnataka and Kerala are few States which allow Candidates from other States to register and participate in their State Counsellings for their Management Quota seats. There is no online site as such. The Counselling conducting authority will open the reservations and activate the link. That is when we will have access to registration. For that we need to wait for the respective State Authority to begin its NEET State Counselling Registration process.

As we know that KEAM rank is calculated by considering 50% of KEAM score and 50% of board marks. Now it is difficult to calculate any particular rank as it depends on various factors like no. Of candidates appeared, highest marks obtained etc. And these factors are bound to change every year.
As you have good performance in 12th and you requires your rank in KEAM under 15000, so you should try to score in the range of 270-320 marks.
