Please contact the KEAM office and ask the solution. Their helpline is good. If your caste is entered wrong and you are applying for reservations then it will matter. Please try contacting the CEE office.

No you don't need to do so.Only you have to provide your caste certificate at the time of counselling.

According to the eligibility criteria of KEAM, reserved category candidates need to obtain a minimum of 45% in maths and 45% in physics, chemistry, mathematics altogether. Hi 

Yes, you can apply in other states as well but you have to check the eligibility criteria for that, because Karnataka and Maharashtra offer admission on the basis of the NATA but not in their government colleges. They also demand domicile.

No, you will not get any refund after getting application rejected by KEAM. You have to be specific while filling the application form with guidelines prescribed by KEAM. 
 
Here, you can check the cases in which application can be rejected without refund.
