Dear Kokila, you need not apply for NEET Admit card. When the admit card is released from April 15th, you will have to login at nta.ac.in with your Login credentials. You can download your admit card. Now the admit card has 2 pages and the 1st page has your details.

You need to fill in your registration number itself. There would not be a separate enrolment number. Your registration number would be the same as your user ID. So kindly provide that in your application forms.

Yes you can definitely take class 10 admit card as your id proof. But if in the guidelines, it is clearly mentioned to take some other id proof you should take that. But its better to take the id proof which you have mentioned in youy application form.
If they have mentioned that name sjshou be entered as stated in the class 10 admit card, you should do that only and carry your class 10 admit card as your id proof only. It might be a problem if you have entered the name as per guidelines
