For admission in IGIMS, Patna the cut off in 2017 for the OBC Category was a rank of 9255 and 558 marks under the State quota. Considering your marks and your category, you do not seem to have much chances for this college.
The admission fee is Rs 80,000
Here is a list of all government medical colleges in Odisha and their respective cut offs in 2017 through the state quota. The cut off for 2018 will be released only after the whole counselling procedure is conducted. Till then, you can expect a similar cut off.

Cutoff of NEET 2018 for Karnataka has not been released. Also, cutoff vary from college to college. Based on the previous year cutoff, For admission to Government Medical colleges in Karnataka under state quota 400+ would be a decent score.
