Yes, to apply for state quota, you need a domicile certificate as a proof; so apply for a domicile certificate. Other documents that you need for state counseling are as following-
1)NEET admit card
2)NEET rank letter
3)ID proof
4)For DOB proof-class 10th certificate /marksheet

If you are referring to the aiq counselling then theres no need to fill different. Once you have registered for counselling and paid the fees you can fill choices for both bds as well as mbbs.

You have a good score in NEET 2019. You can get admission in private college for sure. You have also good chances of getting government colleges. so do try for a government college. I cannot tell the exact college you can get as the cutoff varies every year.
