While filling the NEET Application form, one has to follow certain rules guided by National Testing Agency (NTA). It is Not mandatory to mention Name and date on Photograph. So you can either make it or not , there is no problem with that.  But you should carry passport

The  correction window for the NEET UG 2020 application form will be open from the 15th of January,2020 to the 31st of January 2020. In  that window, you are free to make changes in your state code of eligibility. However, you should also carefully fill any additional information
