I assume you belong to unreserved category.
As you mentioned with UR category and  486 marks, your chances of getting a decent government college in your state through state quota(85%) are quite low except one or two colleges like-
  • Dr. Ram Manohar Lohiya Medical Institute, Lucknow-previous cutoff 469

The cutoff marks depends upon which category you belong to. Since it is a private medical College there is only 85% state quota and you will only be able to apply for this seats if you belong to Karnataka state. Since you haven't mentioned your category answering your

If you have scored 524 marks then why do you want to go for the private colleges. you have the chance to get the govt. seat if you apply through the state quota counselling. So there is no need according to your marks when you can have the govt. medical

You have not mentioned your category, so it is quite difficult to do exact predictions. However in most states, with OBC category and 364 marks, your chances of getting a decent government college in your state through state quota(85%) are quite high as cutoffs for OBC are usually around
