You can get into lmniit but you can't get top branches like cse or ece as the cutoff is high.

Don't worry about it wait till counselling ends.

The condition which you are suspecting to have happened is not at all possible there is no human intervention in rank entry it's all computerised centrally and no changes manually or whatsoever can be made in the system, and are always accurate hence the possibility of a wrong

You can join preparatory course if you are willing to do but even then it's difficult as the competition is cutthroat

The JEE advanced cutoffs are very high .

Based on previous year statistics the sc cutoff was close 1200

In entrance exams the category rank is considered first as you know in category the competition is comparatively a bit less as compared to the total competition. So as per the trend category rank is considered.

In Bihar getting PMCH is tough as the cutoff rank was around 4000 last year. You may easily get NMCH or IGIMS Patna. The cutoff is expected to stay around 6000 to 8000.
