Just follow the DTE counselling procedure. They will ask for the score card only after it is made available, not before. You just have to enter other details. It is after the cap rounds and after you are allocated a seat, that you have to upload the score card photo.

You have not a too good score in NEET 2019. You have negligible chances of getting a government college. You can try for semi government college but there also chances are less. But you can get a seat in a private college for sure. So do try accordingly.
