If you're willing to apply in the top colleges of India I'd say that a score above 550 can fetch you a seat. If you have scored decent marks like 500-520 plus I'd say you'll easily get a seat in a decent government college of your choice.

according to your percentile, the probable rank is low. With this rank, it is difficult to get admission in any of the nits or iiits,

It depends upon the JEE Main and JEE Advance cut offs, If You manage to qualify the exam by achieving the cut off, You can apply for JOSAA Counselling. It should be noted the cut off for EWS/SC/ST/FEMALES/OBC category is lower than the general category

You seem to have very less chances of getting a government medical seat though the cutoff depends on category and increase with the reservation category but chances are still less as per your marks secured. You may apy for dental/ deemed/ private medical colleges.

But usually the cut off for General category is above 400 and for reserved categories it is above 400.Some of the Medical colleges Neet last year cut off are:

If you go for ALL INDIA quota then you will get low to mediocre level colleges throughout the country. You haven't mentioned your state so I'll suggest you to check college predictor once to make things clear.
