Since you have registered yourself with a changed name, your new name will be visible on the admit card. But keep in mind that your name has to be the same on your official government documents. Else it can create unnecessary problems during the further procedure of examination.

2. Click on the link for 'Admit Card/LOC/Centre Material for Board Exam 2017 (School login for Regular Candidates)' on the top of the recent announcements section.
3. Click on 'click here to proceed' on the new page that opens to go to the log in page.

When you will download the admit card on A4 size sheet the self declaration certificate will by default come on the same page. And if you have it on two pages also it should not cause you any trouble as it is just for safety purpose.

You just need to declare yourself COVID free for safety reasons on the centre. So that is not a major issue.
