Based on your profile and rank, you have low chances of getting into JNTU or Andhra University. However you have decent chances of getting a seat in SV University as their last year cutoff was around 4000 for BC-D category. Other colleges you can choose from are:

With 582 marks amd belonging to unreserved category you can get medical seat in andhra medical college throygh state quota as well as all india quota. As per previous the cut off of ap for general under AIQ was 552 marks and under stage quota was 611.
