NEET UG Eligibility Criteria 2020
Candidates must at least be 17 years of age by December 31, 2020. There is no upper age limit . Candidates must have qualified Class 12 or equivalent examination with Physics, Chemistry, Biology/Bio-technology and English as subjects.

For Du one form is filled for one course. Depending upon the cutoff you can apply admissions in the colleges. under the Delhi University, you don't need to fill different forms for one course. However for different course one needs different forms.
