I have recently read a news that one can shift from bams or bhms course to mbbs after finishing one or two years of that course with a good percentage.. Is that really possible..??

The thing you are talking about is probably a brigde course which might be offered after completion of BDS/BAMS/BHMS. This is not official yet, at the government's decision is still pending in order to weigh pros and cons.

And to answer your question, yo u will not be allowed lateral

With this score, your rank would be between 246358-262168

It is good score, but it is difficult to get seat with this score.

Cut off is very high this year. So with BAMS, BHMS course it is difficult to get seat.

You can go for Physiotherapy

The general cut offs for ST Category  for NEET 2020 is 146-113.  With a score of 158, unfortunately you cannot take admission even if it is a Private College. The score is quite less. But if you have decided about studying Medicine, you have to reappear for NEET in 2021

With a score of 249, it's quite difficult for you to get a seat in Government Medical College for both medicine and AYUSH courses. But, you can opt for Private Medical College or Deemed University of your state i.e Gujarat.
