It's quite difficult to predict that whether you will get seat or not as cut off marks for medical colleges keeps changing every year depending upon various factors like number of students that appeared in the exam, kind of marks obtained by them , toughness level of the examination etc.
But when we go through previous data then it suggests that with 421 marks you hold a low chance to get a seat in a Government Medical College
For example, In Neet 2019, the lowest marks that got a seat in Government Medical College for sc category under Bihar state quota was 449 marks and it was for Government Medical College, Bettiah
With your marks you have chances for govt quota seats in private medical college and deemed universities

Hello dear student,
The marks that you have obtained are really less. so it will be difficult for you to get admission into any of the government medical colleges or even in government dental colleges. The chances of you to get admission under management quota seats are also nil because of the low marks as this year cutoff have increased enormously high . I hope this was helpful.
Thank you

With your NEET marks which is 162 you'll be eligible for the state level counselling but the chances of you getting even private medical colleges is really low even with your category rank.
You can get direct admission in private medical colleges but that will cost you a lot since your rank is pretty low. You should take a year drop and prepare for NEET 2021 and score well in that so that you can be eligible for some good medical colleges.
