No, neet PG 2021 examination has not been conducted yet, the examination date for Neet PG 2021 has been announced according to which it will be conducted on 18th April 2021.

NEET PG 2021 application form is expected to be released this month i. e February 2021  in online mode at its official website i. e nbe.edu.in.

Once the application form gets released you can fill it following the below steps :-

* first of all you need to visit the official site i. e  nbe.edu.in and do the online registration to generate your login credentials

* then you need to fill the application form, you need to fill the following details -
-) your personal details like name, your date of birth etc
-)your qualification details
-) your internship details
-)medical registration details

* after filling the form you need to select  the city for  Neet PG examination centre

*then you need to upload scanned images. You need to upload -
-) your passport size photograph
-) your Signature
-) your left-hand thumb impression

* you need to pay the application fee and finally take a printout of the confirmation page for future reference

For pending all year mark sheets you have to apply  to the authority of the respective University /Board from where you have passed the earlier qualifying examination for admission in applied economics.

EWS certificate is applicable only for those general catefory candidates ie., candidates who are not covered under any reservations (such as OBC, SC, ST and except PH and Ex-servicemen criteria) and belong to economically backward sections.
And there is only one EWS certificate which you can get from your state authority. In India there is no such distinction such as state and central EWS certificates. So the EWS certificate you have got from your state's concerned authority (eg. Tahsildar office),  is valid all over India. The validity of EWS is one year.

Please mention about the board from which you have done the course. Also whichever the board is you should contact them because of you have got acknowledgement number then you should have got the OPT. Make sure your number is on and try to re apply and if the problem persists then you should contact the authorities regarding the matter.

Since your medical registration is under your new name, you may use that if you have valid name change certificate and degree certificate with new name.You may not face any issue while applying , the important phase is document verification  Now it is not possible to say how the exact scenario would be during the verification process during PG admission counselling because it depends on the how the counselling authority and the specific college officials attend to this complication but , as far as you have all the valid documents and proofs related to your name change , it will be fine. They may ask for many kinds of verification until they get convinced so be ready for that. You have changed your name legally, so nothing to worry, you PG goals will not be affected. Now on the other hand if you posses only the name change certificate , with you old name still existing in all other academic certificates and other documents (aadhar licence ,MBBS,12th, and 10th etc) then ,you should register with old name and later during PG admission show them proof for new name and change accordingly. S to be on the safer side , you can try inquiring about this to any government official in the medical field ,best is to ask directly in a government medical college or try contacting the NEET PG authority through their queries and helpline infos.
