It is always helpful and better to get one's own caste certificate as it makes the process easier. Most of the colleges would not accept the caste certificate of a family member and require that of the person applying, so i would advise you to apply for your caste certificate online.

Officers competent to issue Caste/Tribe certificates for JEE advanced are:
1. District Magistrate / Additional District Magistrate / Collector / Deputy Commissioner / Additional Deputy Commissioner / Deputy Collector / Ist Class
Stipendiary Magistrate / City Magistrate / Sub-Divisional Magistrate / Taluka Magistrate / Executive Magistrate / Extra Assistant Commissioner.
2. Chief Presidency Magistrate / Additional Chief Presidency Magistrate / Presidency Magistrate.
3. Revenue Officers not below the rank of Tehsildar.
4. Sub-divisional Officer of the area where the candidate and/ or his family normally reside(s).
5. Administrator / Secretary to Administrator / Development Officer (Lakshadweep Island).
Certificate issued by any other authority will be rejected.
Since you don't have time to get it signed upload this document now and you can present the signed document by SDO or SDM or any authority listed above at the time of counselling later.

A caste certificate simply states one's caste while a caste validity certificate validates the caste certificate. It is very important to renew your caste certificate. If you fail to do so then you won't be considered under caste category. Institutes will treat you like an open candidate. Also, you won't be able to get certain perks which caste candidates get.

The instructions for caste certificate states that the certificate must be issued by an authority of the state of West Bengal. As you have mention , DISTRICT WELFARE OFFICER, KOLKATA and Ex-OFFICIO JOINT DIRECTOR , BACKWARD CLASSES WELFARE , is absolutely fine.
Your caste certificate will be acceptable.

I would suggest you to fill up the application form before 19th August 2021. No doubt, you will be able to fill the application form till 05-09-2021 but with late fee fine.
If you fill the form between 20th August to 28th August 2021, you will be charged with late fee fine of Rs. 500. If you fill the form between 27th August to 31st August, you will be charged with Rs. 2000 as late fee fine. And if fill it later by 5th September, you will be paying Rs. 5000 as late fee.

I am guessing that, your name is same on your 10th and 12th certificate and different only on your domicile and caste certificate
If your name is different on your domicile and caste certificate then you still have time, you can get new domicile and caste certificates made with correct name.
In case, if your name is different 10th on 12th certificate then you can get an affidavit done mentioning that both the name belongs to the same person i. e you
Also, one needs to fill name / parents name /dob etc in neet application form as per 10th certificate.

ALL INDIA QUOTA is 15 % total. So in this as well there is reservation for all the category. OBC/ SC /ST separate quota is their for each of them. You dont need to worry for this.
Reservation is mandatory in both 85 % and 15 % quota. Hence, there is no doubt and confusion for 43 % pcb. For general category only, the requirement is 50 % in10 + 2 higher secondary from a recognized college / university.
