As you mentioned you are clear with the concepts of subject.So,all you need to fetch good marks in JEE mains is practice.

No, there is no need to fill another application form for another merit based course instead you can mention both the course in same application form of DU until and unless two course are on different basis like one is entrance basis and another one is merit basis.
