First of all, no matter which college or University you choose in the world you can't become a doctor right away after 12. 

After passing Class XII, one has to study for 5 years with 6 months of compulsory internship to get the degree of MBBS.

Yes, you can take any of the subjects mentioned except physical education. Moreover, you rightly said that there will be a 2.5% deduction because you have not studied physical education. For sure you are welcome for any queries.

Firstly candidates must have passed 12th grade from recognised University and must be minimum 20yrs age. Hence you shall not ve eligible for NId DAT exam.
