1. allotment letter

2. registration slip (application form)

3. AIIMS PG admit card

4. MBBS/BDS degree

5. marksheet of MBBS/BDS professional examinations

6. internship completion certificate

7. provisional/permanent registration certificate issues by MCI/state medical council or DCI./state
