You need to be eligible to obtain a domicile certificate. For that you need to be a resident for past 5 years in Madhya Pradesh.
The documents required will be:
1.) Ration card.
2.) Address proof- passport, ration card, telephone bill, etc.
3.) Transfer certificate from school.
4.) Application form in the given format.
5.) Proof if owning any property in MP.
6.) Identity proof.

if your application is under the process already for verification then I can make verdict that it'll get passed or rejected.
My suggestion for you is that you should inform the issue to the authority and get guidance and also submit all the documents for corrections and make a affidavit mentioning that you'll submit all the correct documents with a date which will be provided by the authority.

To get any state's domicile benefit, you need to have either permanent residency of that particular state or you have completed your high school from that. These two conditions or either of the two can help you in getting domicile benefits and state quota advantages during neet counseling. During registration for neet you can choose Assam to be your domicile state. But make sure to choose only one cause more than one is not considered valid.

It completely depends upon the state you are talking about.
In order to get domicile of any state you need to fulfill its eligibility criteria for the same
For example,
You can get domicile of Delhi and be eligible for 85% state quota in Delhi by just passing your 11th and 12th from Delhi.
Whereas,
If I tell you about you UP then
You can get domicile of UP and be eligible for UP state quota by fulling any one of the below criteria:-
You must be native of UP and your father should be a Resident of the state of UP.
Your parent/s should have lived UP for at least 3 years.
Your parent/s should be working with the Government of India.
If you have passed your 10th and 12th from the state of UP, then you don't need to furnish domicile certificate and you will be eligible for UP state quota
If you have have passed either your 10th or 12th UP, then you need to furnish UP residence proof of your parent/s in order to be eligible for UP state quota

This is the main process. If you are not domicile in UP , but your husband live in UP then may be you will transfer your all original documents and certificate in UP. For this reason you are a domicile in UP if all documents convert into your husband legal wise.

Your father is in army so generally you are moving around the India. And don't worry about the residential certificate. Because if your father live in Assam then you are domicile residence in Assam. Either you can't give the 10th and 12th from Assam but it's not the problem. But if your father is not from Assam then you can't apply for residential certificate. Where your father born this is your residential certificate.
Although your question is not clear dear. You may be born in Maharashtra but your 10th and 12th class board examination from where it's not clear from your above query. And may be you want to know the domicile residence in MP or not.
Look if you are staying a City for almost 10 to 15 years and given 10th 12th class examination from there then you are applying for domicile certificate. But if you are not constant living in one area then where you born or where your parents born or live you can apply domicile certificate from there. I hope you are clear the problem.
