In West Bengal:
Here you cannot get a MBBS seat in Government Medical College but you can get a Govt. Quota seat in a Private Medical College.
In Assam:
Assam has lesser cut offs and so you can get a GMC in Assam. This is all based on 2019 data and cut offs might change. First of all congratulations for your rank. There are chances for you to get seat in private college in Karnataka. But I cannot assure you, that you will definitely get a seat. This year competition is extremely high, even with 550 marks there are not 100% chances for that person to get a seat. In Tamil Nadu closing government college cut off was around 601 for OC category (This information was provided to show the level of competition), Incase if you are not able to secure a seat then go for deemed colleges or pursue from abroad that too only if you wanted to pursue immediately. Or else take NEET again.

If this is regarding NEET, then Yes. Let us say for example, you are a permanent resident of UP but studied your 11th and 12th in Delhi. Because you are UP's permanent resident, you will have the State domicile and so you can participate in UP State Counselling. And because you studied your 11th and 12th in Delhi, you are also eligible for Delhi State Counselling. So because you are eligible for these 2 States Counselling, you can apply when these States open their registration process. This is just an example. But in a broader spectrum you can apply. The cutoffs vary with factors like number of seats available , number of students applying , the overall NEET results, your ranking, difficulty level of the paper etc. Hence , nothing can be predicted for sure. In your case for SC category , 400+ is a safe score for a decent college based on previous year cutoffs. Getting a desired government seat may be a little doubtful also since you are from AP you can't claim home state quota reservation in Hyderabad since it is in Telangana state , you can claim 85% quota only in AP and in Hyderabad you be will considered under the 15% quota . So you have to attend the counselling and see.
