There are many specializations which you can get. The specialization offered in India through neet pg is anesthesia, radiotherapy, orthopedics, obstetrics and gynecology, general surgery, psychiatry, dermatology, general medicine, pediatrics , opthalmology, ENT, preventive and social medicine, pharmacology, microbiology, forensic medicine.
600 is a decent score. You might get M.S ENT in govt college. However, this may change depending on the cutoff and various factors.

Hi, the information you gave is insufficient. Are you planning for a PhD or are you planning for a job in USA. As the job visa are hard to get (not impossible). For a PhD there are good universities that you can get into are Johns Hopkins University, Emory School of medicines, etc. If you still have doubts feel free to comment here.

Conducting NEET 2021 twice a year is still under consideration. Our education ministry is thinking about this topic and no confirmation is made till yet. So, it is not sure whether it will be conducted twice this year or not.

Postgraduate degree holders in Ayurveda will now be allowed to perform a variety of general surgery, ENT, ophthalmology, and dental procedures. The government has amended the Indian Medicine Central Council (Post Graduate Ayurveda Education) Regulations, 2016, to introduce formal training in these procedures as part of the curriculum for PG students of shalya (general surgery) and shalakya (diseases of ear, nose, throat, ENT, eye, head, oro-dentistry) specialisations.

Yes its a post graduation course, and you need to complete your graduation also and need to crack the NEET PG entrance examination. You also need to complete your rotary internship before 31st of March of the year you are appearing for NEET PG.

Since you have not given us your rank in your NEET UG we will assume you have done fairly to be allotted a DNB ENT seat in the second round of counseling. With MD Pediatrics in the 3rd round will be difficult but there is slight chance in ENT. DNB may feel like you are doing a doing a lesser program but gathering knowledge is the same for both. Your keenness to learn will be rather defining in both, also find out if the college in Secundrabad has a high patient volume – this is most important when it comes to learning.
What MCI says about MS ENT : The purpose of MS ENT is to standardize Otorhinolaryngology teaching at Post Graduate level throughout the country so that it will benefit in achieving uniformity in undergraduate teaching as well and resultantly creating competent ENT Surgeons with appropriate expertise.
What MCI says about DNB ENT: Practice the specialty of Otorhinolaryngology ethically keeping in mind the requirement of the patient, his community and people at large. Demonstrate sufficient understanding of basic sciences related to Otorhinolaryngology and be able to integrate such knowledge in his Clinical practice. Diagnose and manage majority of conditions in Otorhinolaryngology
So MS is about treating and surgery and DNB is about treating only. Now if surgery is important for you then you need to take a chance, if not DNB and later think about MCh if needed.
NB – If you ever intend to take up teaching then MS or MD is needed.

Yes with a state rank 262 you can get a government seat for Orthopaedics or ENT or general surgery if not in the first round but definitely in the second or third round of counselling.
