Sorry to say that with a rank of 18,000 in ews category it is difficult for getting a seat in an Nit as the total seats for ews are under 3000.

To be honest thinking of securing any government seat under All India Quota seems very difficult for you at this rank. As the competitions are high and the seats for government colleges are limited, so the chance is very less. Your category will also not fetch you a good seat like other categories.

The cutoffs for NIT are influenced by various factors which include total aspirants, difficulty level of the exam, number of seats available for admission,etc
Based upon the ranks that you have mentioned, and considering the previous year cutoffs, it will be tough for you to get admission via your EWS category rank.
The cutoffs may very closer to the EWS rank that you have secured, and the competition will be to tough.

Sorry to say but it seems difficult to get government medical seat with these score in Maharashtra and even in other state through All India Quota. EWS category candidate must score atleast 500 and above to get government medical seat in their state and as per last years cutoff EWS category candidate needed 560 and above score in NEET to secure their place through All India Quota.
