Yes you can upload but when they recheck the certificate may be they not consider your certificate so

If possible then do corrections first on your certificate then upload . It is good for you .

Address is in what is different?. If your state is same in both the form I don't see why JEE officials have a problem with that. Still to be on safe side upload a self declaration in lieu of the certificate then upload it at the time of reporting
