Yes, you can take 12th admit card as Id Proof, along with this you need to carry NEET admit card with self declaration(undertaking) duly filled in A4 paper with photo affixed and unsmudged left thumb impression(signature to be done only in the presence of the invigilator), mask, gloves, a a personal sanitizer of 50 ml, personal transparent water bottle,additional photograph same as uploaded on the application form to be pasted on the attendance sheet. Neet eligibility criteria says that, to be eligible one must have passed in English and in both the theory and practical of PCB i.e Physics, Chemistry and Biology with at least 50 % in aggregate of PCB ( in theory +practical) in 12th examination if belong to general category , or at least 40% in aggregate of PCB if belongs to Obc/ Sc /st.
Now, you belong to SC category and

= 158
=(158/300*100 )%
=52% , so regardless of the category you belong to you fulfil above aforesaid % requirement and thus you are eligible for neet examination

If you have passed ssc and hsc from gujarat and get medical seat in gujrat through all india quota or state quota , there is no need of providing domicile certificate , as you 10th -12th marksheet only refers that you are a gujrat students.

With that percentage and marks, its hard for you to secure any position in any government college through GUJCET. You might still apply for any private college out there because they can still take you as per the cut off declared. You haven't stated your category so can't comment much but in general category, you might not.
I think you may have to fill up an entirely new application, I'm sorry.
