after 12th with psb can I do bsc in pathology

Yes you can do Bsc in pathology with PCB subject.

For UG Courses: Minimum educational qualification of 10+2 passed with PCB group is required for admission to undergraduate courses in Bsc pathology.

If you are changing your name by help of law. Means change your name by affidavit then you will be use your new name. And your 12th school said the right thing. But if your college is not ready to your new name then you can show your affidavit

There's not a problem at all if you have any backlogs in your graduation, but all of your backlogs must be cleared or otherwise you you won't be considered a Graduate. Now, you are absolutely eligible to to fill up SBI form but you must have get your mandatory
