Is would be better if you apply for both ie. All India and state councilling if you dont get seat in all India than you can have a change of getting seat is state quota so apply for both.

If you do not get any seat during AIQ MBBS counselling rounds, then you will have to apply for BDS rounds. The admissions will not be automatic. You have to manually apply for the course where you want to take admission in. There will be rounds for MBBS,
