Last year the course at Kasturba gandhi college started in the end July. For this year there is no official announcement regarding the course start. It is possible that the course starts by 25th of july, still wait until official announcement comes.

See as per your Marks-497 it is difficult to get a Government Medical Seat even if you have a state quota and another quota if applicable. You should have maintained a score above 550 to be on the safer side.
You cannot get into any medical college with this mark.
It is not possible to get into ESIC IP Quota.

You may try for private colleges where The rank up to which a college will it will offer a Medical MBBS Seat depends upon the College itself and varies from college to college. You must enquire it from the college you desire to seek admission into.
Some of the low-cost private colleges are
1.Terna Medical College, Navi Mumbai The fees is around 32 lakhs
2.CCM Medical College, Durg The fees is around 43-45 lakhs
3. Raipur Institute of Medical Sciences, Raipur The fees is around 27 lakhs
4. Kempegowda Institute of Medical Sciences, Bangalore The fees is around 1.5 lakhs
5. Sri Aurobindo Medical College, Indore The fees is around 6-8 lakhs

For MBBS admission into any college you have to apply for the counselling first. Based on the marks you have obtained in the NEET you will be given the admission. Therefore first of all apply and appear for the counselling rounds. You can apply for the colleges through the state counselling or the all India counselling.

AIIMS Patna was founded and started it's academic schedule in 2012. It's a fairly new institution, currently mentored by AIIMS New Delhi. Being an AIIMS, it will definitely be one of the top institutes within the coming 5-6 years.
It usually takes a medical college 5-6 years, that is, until the first batch passes out to gain a strong foothold.
KMC Manipal, on the contrary has been a top private college(the second best private institution in the nation, only behind CMC Vellore). It costs a lot more to study at KMC. However, currently, it is better to go for Kasturba Medical College, Manipal if money is not a problem

You have not mentioned which category you belong to and also whether you belong to state quota or All India Quota. If you belong to All India Quota then there is less chance of getting a seat with a score of 441 marks. If you belong to State Quota then you have chance chance if you belong to the any category reservation. To predict the colleges check out the link.

As you will be knowing that each campus has a perk of its own. As far as teaching and learning are concerned, both the colleges are well stacked up with amazing teachers, activities, academics, training etc. The contrast is in the fact that KMC Mangalore has a dispersed campus. Unlike Manipal, we have to interact with and travel through a city while we study.
I personally like Mangalore because, along with getting done with the course, I feel that one gets to learn to live an independent life here Science can be mastered anywhere with effort with time, but the Art of it must be cared for by people of dedication and experience, it must be sown, grown, tended and protected like a plant, allowing its roots to grow strong and deep, before it can spread its branches like a banyan tree, providing shelter to everything which take refuge under it.
*To me, Manipal or Mangalore, I would recommend Kasturba Medical College even over Govt Institutes for MBBS training.

Getting Kasturba medical college through AIQ is difficult as thisnis the first deemed University which is allocated along side IMS BHU and AMU.
Through deemed University quota as well the cut off is high.
Attend the state counselling of Karnataka state in order to have a great probability of getting a seat in KMC, Mangalore or Manipal.

mop up round is held state wise as well as All India. If you want to participate in the 15 % All India quota counselling mop up round, you have to register yourself with MCC from the very first round of counselling. If you want to participate in the state mop up round, then you have to register yourself with the state counselling.
Manipal University is a Deemed University so you needed to register for Deemed Universities MBBS programme. I hope you had successfully registered for that.
I would like inform you that the defence quota is at a state level and not at college level. Certain states have provisions for this quota. And there the reservation is only upto a 1%
However Individual colleges do not provide such reservation.

As per last year cutoff college was alloted to 500+ marks, for mop up it was around 51, chances increases for state quota as it was 430+ so you seems to have tough chances of getting a seat this year but none the less cutoff changes every year and you may apply for mop up round as it give some hopes. It is difficult to tell exact chances.
