There is no any management quota seats in Kasturba Medical College, Manipal. The admission is only based on NEET Entrance Exam score.

If or not you can apply for counselling for state quota admission in other states will depend upon the rules of the respective state.
As for Delhi, yes you may apply for admission in Delhi, but you will be considered for admission in Delhi MBBS courses only under All India seats and not those reserved for canidates of Delhi Region.

Official data for fee structure of 2018 hasn't yet been released, but it is expected to remain on the lines of 2017 admissions. In 2017, the annual tuition fee for Kasturba Medical College (KMC) Mangalore for Government Quota seats was INR 77,000/-.
As per the second part of your question, I will again give you 2017 lowest score which got admission on Government Quota seat in KMC, Mangalore, to give you an idea of marks needed to get government seat. In 2017, the last rank which got admission on government seat in KMC Mangalore got a score of 397 (out of 720). However, note that the score needed changes every year with fluctuations in difficulty level of the exam, preparation levels of other students, number of aspirants, etc. So, I suggest, aim higher so as to fall in a comfortable zone and secure your chances of getting an admission.
