Hiii Anupama!!!! To get access to previous years questions of SLAT, there are many online books available. You can also buy books of Lexis Nexis or Pearson, they provide with the previous years questions. Many coaching centres also give question banks. Hope this helps.

you dont need to give any middle name of your father name if your hsc/ssc certificates has not it. always give your name, father name according to your ssc certificate. and there are many people in india who dont have a middle name. just keep the column of
