The application form has released on June 8, 2021. They have released this form on the official website, mahacet.org 2021. The Last date to submit the form is July 8 to 15, 2021.they will release the 2021 exam date  soon. Mainly the documents required are

But MHTCET is for engineering,pharmacy and agriculture courses. For BSc cardiac technology NEET score is preferred. However it also depends on the institute you are applying. Some institutes have their own entrance test.
