The prospectus tell you about the Achievements of that Particular College or University, Its Ranking, Courses the college offer, Fee criteria etc. This Prospectus also contain Admission form to be filled, Thus colleges and universities

yes, physical education can be included in best four. but there are certain rules for various colleges and univeristies.for eg: if you include Ph.E while applying for DU 2.5% will be deducted from the total percentage of your best four subject.

If you have scored around 85 or 95 raw marks, your composite score will go around 750 and would get you a percentile somewhere around 99. Since you are very close to 99 percentile, you have high chances of getting a good college.
