Cut off neet of government college and private colleges  change every year. So is with government colleges kerala.

Cut off depends upon state, number of students appeared , previous year's cut off , normalisation method etc.

Exact marks can't be estimated but for OEC category,  for getting mbbs

i have severe illness and i need atleast 5 months to recover from it will my medical College provide me medical leave and will it allow me to sit in examination (currently first year mbbs student)

No medical college will allow leave for 5 months.

As you are first year MBBS student you must be well aware of 75% attendance rule.

Basically required attendance is 80% and there is 5 % relaxation.

This rule is strictly followed , In case of genuine reason sometimes

You can appear for neet again. If you get better score next time then only you will be eligible for Ayush or mbbs course.
