You will get direct admission only in private colleges and for that just do some Google research saying "private colleges offering BOT" in your preferred state. It'll show you top private colleges of the place and then you can yourself decide which suits you by seeing all eligibility criteria,

According to the the previous year cut off, there are 15  medical College in Tamil Nadu whose MBC category cut off for state quota was less than 353 hence you have a fair chance to get medical seat in those 15 colleges. I am adding the link below

After 12th you can go for Ayush courses like BAMS, BHMS etc. if you don't want to go for MBBS & BDS or you can choose other careers option in Life sciences and some of the courses that can be considered for graduation are:
