As per previous year cutoff, at ESI Post Graduate Institute of Medical Sciences and Research, Bangalore and Kodagu Institute of Medical Sciences, Madikeri had the lowest cutoff marks at 368 / rank - 104690 and 104137 respectively. So accordingly, cutoff ranks can be like this, this year too.

Hope you are well. Sorry to say but a mere score of 158 isn't enough to get you a seat in good government colleges,  there is a huge competition in the NEET exam.

first of all please let us know your entrance exam type in the comment box and your category also. Nowif your entrance exam is NEET 2019 THEN based on previous years cut-offs it is hard for you to get any government college through all india counselling. Even if

You are from AP, So you have to get a seat through all India quota in Tamilnadu medical colleges. If your category is SC or St you can get a seat in most of the govt. colleges of Tamilnadu for your marks. For UR category you can get

As per your interest it seems that you have mentioned about NEET rank. Still, with this rank and category rank it will be little difficult to get a MBBS seat considering last year cutoff's.
