In order to apply for medical seats in national pool i.e all India quota, you need to apply for counseling which is conducted by medical council of India.
Via all India quota you can get admissions in private colleges, deemed universities, ESIC colleges, AFMC , government medical colleges.

You need not spend a crore for a medical course. Basically you will have categories basically the government quota and self financing. The fee for the government quota will be around 4-6 lakhs per year. And self financing will cost you around 12 - 15 lakhs per year.
